It depends on how the airline ordered the plane. As an example some 757s have a mid cabin door and no window exits while some have window exits and not mid cabin door. Its just like if you get leather or cloth for your car, planes have lots of options and configurations to.
Or it could be based on the model (Size) of the 767, like the A319/A320/A321,
The A319 has 1 over-wing exit
The A320 has 2 over-wing exits
The A321 has 0 over-wing exits, rather it has mid-cabin exits

The planes are the same size just configured to the airlines wants.
It also depends on the # of seats the airline plans for the plane, the more seat the more exits needed!

I think it also matters to airline preference. BA, for example, use a second set of main doors near the wing to break up F and C. A second set of doors on AA and DL, for example, would cut right through the Y cabin, losing seats, whereas window exits do not take away seats (but do somewhat affect pitch and spacing).
On a side note, DL has a few 763s with a second set of main doors. These are ex-Gulf Air birds that DL acquired.
