While it's not a civilian airfield, one of my old haunts, Columbus AFB (KCPM) had three parallel runways.

Yes, Midway has 13C/31C that's the longest runway there. Phil I will give you my passord to Jeppesen E-Link and you can search all the runways in the U.S.
